At Full Circle – End of Life Matters, we work alongside health, aged care, and community services to support individuals and families navigating ageing, illness, dementia, and end of life.
Our approach is doula-led and clinically supported, designed to complement existing care rather than replace it. We work alongside clinical teams and community services to help strengthen communication, continuity, coordination, and support around the individual and family. We believe the best outcomes occur when families, communities, doulas, and healthcare professionals work together.
We provide relational, non-clinical support that helps individuals and families feel informed, connected, and supported throughout their journey.

For Referrers
Referrals may be appropriate for individuals and families experiencing:

Frailty and ageing-related decline

Dementia and cognitive decline

Serious or life-limiting illness

Palliative and end-of-life care needs

Family overwhelm or caregiver fatigue

Advance care planning needs
-
A desire to remain at home for as long as possible
Support can be provided in the home, residential aged care, hospital settings, hospice or alongside existing community services.
Collaboration
We welcome referrals and collaborative partnerships from healthcare providers, aged care organisations, community services, funeral providers, and end-of-life doulas.
Through our partnership with SpencerHealth, individuals and families may also be connected with professional grief and bereavement support where appropriate.
